First Home Care

First Home Care was established in 1991 to provide community-based placements for children and youth with a history of out-of-home behavioral health treatment. Since then, the First Home Care team has developed an integrated continuum of care, including:

This extensive array of wraparound services has enabled children, youth and families to receive the care they need in their homes and communities, thereby reducing the need for more costly and restrictive levels of care and placements.
Our services target those youth and families deemed "hard to serve" due to multiple system involvement or clinically complex treatment histories requiring access to an array of mental health services and supports.
